About the Role
The Battle Creek VA Medical Center is currently recruiting for a Registered Nurse to be our Traveling Veteran Coordinator within Community Care Service.
What You'll Do
- →The Registered Nurse Traveling Veteran Coordinator will be responsible for: Executing a streamlined approach to receiving, triaging, and directing timely, coordinated care.
- →Bringing principles and theory with limited application of critical thinking skills.
- →Assisting Veterans requiring complex care coordination or specialized case management services.
- →Serves as the primary point of contact these Veterans and their families or caregivers and communicates with the health care team while leading complex care coordination and case management efforts.
- →Serves as a liaison for the Veteran between VA facilities and is responsible for managing the care of the Veteran throughout the process.
- →Assumes responsibility for the coordination of care for traveling Veterans either vested in another Veterans Affairs (VA) facility and traveling to the local catchment area, or traveling outside of the local catchment area, focused on patient education, self-management, and customer satisfaction throughout the continuum of care.
- →Supports the medical center's mission to improve timely access to care.
- →Work collaboratively as a member of the interdisciplinary team, makes caring for the Veteran their priority while in the work setting, and demonstrates customer service principles in all aspects of work.
- →Demonstrates positive, effective communication skills and professional behaviors that promote cooperation and teamwork with internal and external customers.
- →Handles conflict and problems in dealing with any consumer group appropriately and in a constructive manner.
- →Demonstrates integration of biopsychosocial concepts, cognitive skills, and technically competent practice while providing care to clients.
- →Assesses, plans, implements, and evaluates care based on age-specific components Other duties as assigned by supervisor.

Requirements
- ✓Citizenship; non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
- ✓All applicants tentatively selected for VA employment in a testing designated position are subject to urinalysis to screen for illegal drug use prior to appointment.
- ✓Applicants who refuse to be tested will be denied employment with VA.
- ✓Selective Service Registration is required for males born after 12/31/1959.
- ✓Subject to background/security investigation.
- ✓Selected applicants will be required to complete an online onboarding process.
- ✓Acceptable form(s) of identification will be required to complete pre-employment requirements (https://www.uscis.gov/i-9-central/form-i-9-acceptable-documents).
- ✓Effective May 7, 2025, driver's licenses or state-issued identification cards that are not REAL ID compliant cannot be utilized as an acceptable form of identification for employment.
- ✓Must pass pre-employment physical evaluation.
- ✓Participation in the seasonal influenza vaccination program is a requirement for all Department of Veterans Affairs Health Care Personnel (HCP).
- ✓You may be required to serve a probationary period.

- ✓IMPORTANT: A transcript must be submitted with your application if you are basing all or part of your qualifications on education.
- ✓Upon achievement of a State license, the individual may be appointed on temporary basis and later converted to a permanent appointment upon successful completion and graduation from the bridge program.
