About the Role
This Registered Nurse position is located in the Medical Service at the West Roxbury campus location of VHA Boston. This position is full-time at 40 hours per week.
What You'll Do
- →Facilitate hospice referrals to home and other facilities including in other states.
- →Collaborate closely with hospice agencies and social work.
- →Provide nursing and clinician education on hospice, palliative care and Supportive CTraC at VA, affiliates, and MA Hospice Federation.
- →Participate in national CTraC community and mentor Supportive CTraC nurses across the VA.
- →Collect data on Supportive CTraC program to ensure value added, improved care, and cost avoidance.
- →Continually evaluate program effectiveness in collaboration with GEC leads.
- →Relocation/Recruitment Incentives: Not Authorized Permanent Change of Station (PCS): Not Authorized Notifications: This position is in the Excepted Service Current and former Federal employees must submit copies of their most recent SF-50, (Notice of Personnel Action).
- →The SF-50 must identify the position title, series, grade, step, tenure and type of service (Competitive or Excepted).
- →In some cases, more than one SF-50 may be required to show a higher grade previously held.
- →This position is covered by a special rate.
Requirements
- ✓Citizenship; non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
