About the Role
About the Position: Incumbent maintains responsibility for program management and implementation of the Defense Health Agency System of Care (DHA MHSOC), Mental Health Caring Communication / Case Management (MHCCM) Program operations. Ensures optimal delivery of safe quality, while maximizing resources and containing costs. This is a Direct Hire Solicitation
What You'll Do
- →Serves as expert mental health case manager and member of multidisciplinary team of medical and professionals to identify high risk and high-cost diagnosis.
- →Designs timeline-based care protocols that ensure high-quality, cost-effective healthcare for patients and beneficiaries under case management.
- →Develops, implements, communicates, and coordinates individualized empirically informed patient treatment plans that address all diagnosed conditions and health concerns.
- →Communicates with treating facilities, both in direct and purchased care to coordinate patient care and transitions.
- →Participates in clinical audits and performance reviews to advance patient safety and care effectiveness.
- →Chairs and coordinates the weekly At-Risk/Multi-Disciplinary Treatment Planning meeting to ensure treatment plans for at-risk and high-interest patients are synchronized among all medical clinics, and all other medical treatment providers.
- →Ensures staff efforts to identify, plan, assess, and coordinate care programs are designed to provide efficient, comprehensive, and cost-effective service for patients, throughout the continuum of care.
