About the Role
The primary purpose of the Patient Flow Inpatient Case Manager Nurse Registered Nurse position is to demonstrate leadership in delivering and improving holistic care through collaboration strategies with others to provide comprehensive nursing for the complex patients in the assigned area.
What You'll Do
- →Major duties include, but are not limited to: Demonstrates clinical competence in the assessing, planning, services.
- →He/she performs an initial and ongoing assessment of patients to identify needs, issues, resources, and care goals, and identifies critical factors for achieving desired outcomes for discharge.
- →Functions as a systems coordinator for the plan of care; monitors progress through the expected hospital course and coordinates discharge planning with the patient and their physician team.
- →Collaborates with the patient and care providers to evaluate and update changes in the management of the patient and the discharge plan.
- →Advocates for fiscal responsibility in the management of care through effective utilization of resources.
- →Establishes ongoing relationships with professional/health related groups within the community.
- →Recognizes complex situations that impact patient care and intervenes using sound judgement, professional knowledge and appropriate conduits to resolve issues.
- →Monitors length of stay in order to meet VHA and National Standards.
- →Reverts to analytical thinking when dealing with events that are unexpected.
- →Educates rotating resident physicians on the essential elements of discharge and the care coordination process.
- →Provides support to the in-patient nursing unit(s) by coordinating outpatient home care.
- →Initiates follow-up as needed.
- →Focuses on problems, initiates referrals to other disciplines and/or services as appropriate.
- →Accurately documents in the medical record patient findings, assessments, and care provided.
Requirements
- ✓Citizenship; non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
- ✓All applicants tentatively selected for VA employment in a testing designated position are subject to urinalysis to screen for illegal drug use prior to appointment.
- ✓Applicants who refuse to be tested will be denied employment with VA.
- ✓Selective Service Registration is required for males born after 12/31/1959.
- ✓Subject to background/security investigation.

- ✓Effective May 7, 2025, driver's licenses or state-issued identification cards that are not REAL ID compliant cannot be utilized as an acceptable form of identification for employment.
- ✓Must pass pre-employment physical evaluation.
- ✓Participation in the seasonal influenza vaccination program is a requirement for all Department of Veterans Affairs Health Care Personnel (HCP).
