About the Role
The Registered Nurse (Nurse Physician Liaison) is responsible and accountable for all elements of the nursing process when providing and/or supervising direct patient care. They assume responsibility for the coordination of care focused on patient education, self-management, and customer satisfaction throughout the continuum of care. They administer medications and procedures per established policies and guidelines.

- →Document tentative plan of care and disposition for patients.
- →Document discharge planning for in-patients on assigned medicine teams.
- →Document Home Nursing/Hospice plans and referrals.
- →Plan for discharge/transfer of patients and completes discharge note in CPRS.
- →Disseminate a list of daily discharges with disposition plans to multidisciplinary discharge team.
- →Oversee the Discharge Expediter and communicates updates throughout day as to patient discharge needs.
- →Serve as a nurse/physician advocate.
- →Serve as an educator to inpatient physicians regarding CPRS and hospital policies and procedures.
- →Provide individualized patient care consistent with identified knowledge of patient's strengths and limitations.
- →Is proactive in identifying potential crisis situations and responds appropriately.
- →Initiate referrals and/or consultations as appropriate to achieve the multidisciplinary plan of care.
- →Function as a consult person to physicians, nursing and other personal for complex patient care issues.
- →Function as an inpatient case manager, insures that post hospitalization follow-up and discharge needs are completed prior to patient discharge.
Requirements
- ✓Citizenship; non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
- ✓All applicants tentatively selected for VA employment in a testing designated position are subject to urinalysis to screen for illegal drug use prior to appointment.
- ✓Applicants who refuse to be tested will be denied employment with VA.
- ✓Selective Service Registration is required for males born after 12/31/1959.
- ✓Subject to background/security investigation.

- ✓Must pass pre-employment physical examination.
- ✓Participation in the seasonal influenza vaccination program is a requirement for all Department of Veterans Affairs Health Care Personnel (HCP).
