About the Role
The Western New York VA Medical Center is seeking a Registered Nurse to work Intermittently as part of the Home Based Primary Care team, located at the Jamestown Community Based Outpatient Clinic. This Registered Nurse provides direct Veteran care and care coordination, as it pertains, to Veterans and their care within the Home Based Primary Care Program.
What You'll Do
- →The Home Based Primary Care (HBPC) Registered Nurse (RN) is responsible for providing competent, evidence-based outpatient care to HBPC patients with oversight of licensed vocational nurses/nursing assistants as appropriate to the setting, and is responsible for case management, assessment, triage, Veteran education, resource coordination, medication and treatment coordination, and care planning.
- →The Registered Nurse will plan and deliver technically competent care in the home of Veterans with complex needs, including health promotion, health prevention, geriatric care, primary care, and palliative care.
- →The incumbent will utilize interventions, which maximize health capabilities and reduces unnecessary urgent care and emergency room visits, per Veteran's goals of care, and makes home visits to Veterans in the program.
- →The RN will assist in directing the provision of nursing education, orientation, competencies and providing quality improvement and enhance outcomes utilization, by integrating outpatient nursing knowledge, skills, abilities, and judgment and is self-directed in goal setting for managing complex HBPC situations.
- →He/she will validate understanding of the rationale for care processes and decisions, and question clinical practices for the purpose of providing evidence-based care.
- →The incumbent will demonstrate competence in HBPC practice and decision-making, deliberate planning, and critical thinking skills while having the ability to cope with and manage competing priorities.
- →Work Schedule: Intermittent, variable day tour (8:00 am - 4:30pm).
- →No weekends, no holidays.

Requirements
- ✓Citizenship; non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
- ✓All applicants tentatively selected for VA employment in a testing designated position are subject to urinalysis to screen for illegal drug use prior to appointment.
- ✓Applicants who refuse to be tested will be denied employment with VA.
- ✓Selective Service Registration is required for males born after 12/31/1959.

- ✓Participation in the seasonal influenza vaccination program is a requirement for all Department of Veterans Affairs Health Care Personnel (HCP).
