About the Role

At Sioux Falls VA Health Care System, Registered Nurses (RN) in Behavioral Recovery Outreach partner with caregivers to reduce disruptive behaviors related to Major Neurocognitive Disorder, coordinate individualized behavioral plans, and guide safe transitions-bringing compassionate, evidence-based support that helps Veterans thrive.

What You'll Do

  • The VA Midwest Health Care Network advocates for a Whole Health System of care in each of the Medical Centers.
  • This is an approach to healthcare that empowers and equips people to take charge of their health and well-being and live their lives to the fullest.
  • As an employee operating in a Whole Health System of care, you will operate in a model with three core elements, seeking to create a personalized health plan for each Veteran.
  • This is done in the context of healing relationships and healing environments and a connection back to the Veteran's community.
  • This aligns with the Veterans Health Administration (VHA) Mission Statement to Honor America's Veterans by providing exceptional health care that improves their health and well-being.
  • Major duties include, but are not limited to: Working with the Behavioral Recovery Outreach team to manage disruptive behaviors related to Major Neurocognitive Disorder using non-pharmaceutical interventions.
  • Collaborating with unit staff and full-time caregivers to discuss behaviors, identify possible triggers, and share helpful interventions.
  • Modeling STAR VA behavioral interventions to front-line staff when applicable.
  • Co-facilitating scheduling of follow-up appointments after discharge in collaboration with the facility and family.
  • Ensuring the behavioral plan is modified as needed and changes are documented and communicated to team members and nursing staff.
  • Providing updates to Power of Attorney, guardian, and families regarding behavior and condition of Veterans enrolled in the program.
  • Recommending and collaborating with potential facility placements; reviewing the Veteran's behavior plan and pertinent medical information and documenting actions in the Electronic Health Record (EHR).
  • Accompanying the Veteran on the day of discharge and providing transportation using a government vehicle; offering education and support to the Veteran and receiving facility staff; attending scheduled community visits to facilitate successful transitions and documenting actions in the EHR.
  • Serving as the point of contact for community placement staff and the Behavioral Recovery Outreach team upon community discharge; coordinating scheduled visits.

Requirements

  • Citizenship; non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
  • All applicants tentatively selected for VA employment in a testing designated position are subject to urinalysis to screen for illegal drug use prior to appointment.
  • Applicants who refuse to be tested will be denied employment with VA.
  • Selective Service Registration is required for males born after 12/31/1959.
  • Subject to background/security investigation.

  • Effective May 7, 2025, driver's licenses or state-issued identification cards that are not REAL ID compliant cannot be utilized as an acceptable form of identification for employment.
  • Must pass pre-employment physical examination.
  • Participation in the seasonal influenza vaccination program is a requirement for all Department of Veterans Affairs Health Care Personnel (HCP).
  • You may be required to serve a probationary period.
