About the Role
Responsible and accountable for all elements of the nursing process when providing and/or supervising direct patient care. Assesses, plans, implements, and evaluates care based on age specific components. Assumes responsibility for the coordination of care focused on patient education, self-management, and customer satisfaction throughout the continuum of care. Follows procedures per established policies and guidelines. Influences care outcomes by collaborating of the interdisciplinary team.
What You'll Do
- →***Preferred Experience: Minimum 1 year Behavioral Health nursing preferred.
- →Community Care Case Management strongly preferred*** Assigned duties but not limited to: The Housing and Urban Development - Veterans Affairs Supportive Housing (HUDVASH) Registered Nurse (RN) executes position responsibilities that demonstrate leadership, experience, and creative approaches in providing complex patient management.
- →Provides patient management, care coordination, and discharge/disposition planning for inpatient and outpatient settings.
- →Assists Veterans, family members, and caregivers with receiving the most appropriate options and services to meet their complex health care needs.
- →This includes, but is not limited to, acute, chronic, multiple, complex, catastrophic, or life threatening illnesses; combat stress, residuals of traumatic brain injury; community adjustment; addictions and other health problems.
- →Coordinates care with multiple providers across all levels and sites of care.
- →The HUDVASH RN develops a plan of care using information from the patient, family, and provider.
- →Bridges the gap between discharge and primary care follow-up.
- →Identifies high-risk patient population and transitions to appropriate discipline (i.e., Primary Care, Home Based Primary Care, Hospice/Palliative Care etc.).
- →Responsible for intensive monitoring, follow-up, and clinical management of high-risk patients.
- →Collaborates with interdisciplinary team members to develop and implement the Veteran Housing Stability Plan.
- →Contributes to the team-based assessment of the ability of individual Veterans to live independently in the community, as part of eligibility determination for Veterans with complex medical and psychological risk factors.
- →Provide recovery-oriented and Housing First services, with the goal of establishing the Veteran independently in the community, at the Veteran's highest level of functioning.
- →The HUDVASH RN collaborates in the development, implementation, evaluation, and revision of policies, procedures, and/or guidelines.
Requirements
- ✓Citizenship; non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
- ✓All applicants tentatively selected for VA employment in a testing designated position are subject to urinalysis to screen for illegal drug use prior to appointment.
- ✓Applicants who refuse to be tested will be denied employment with VA.
- ✓Selective Service Registration is required for males born after 12/31/1959.
- ✓Subject to background/security investigation.

- ✓Participation in the seasonal influenza vaccination program is a requirement for all Department of Veterans Affairs Health Care Personnel (HCP).
