About the Role

St. Cloud VA Health Care System is seeking a Primary Care Physician Assistant to care for Veterans as a member of the Patient Aligned Care Team (PACT) in St. Cloud, MN. This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. What You'll Do

  • The Primary Care Physician Assistant (PA) position is within the Primary and Specialty Medicine (PSM) Service Line.
  • Duties include chronic disease management, preventative health care and evaluation of episodic acute illness to a predominately male population.
  • FUNCTIONS OR SCOPE OF ASSIGNED DUTIES: Duties as a Patient Aligned Care Team (PACT) provider: Completes assessment and documentation of medical history, diagnosis, and plan of care.
  • Initiates, orders, and evaluates plans of care for panel of outpatient clients.
  • Conducts follow-up exams for mediation management and monitoring.
  • Provides patient/family education regarding disease process, risk factor, symptom control, treatment (including use of medication).
  • Consults with other providers and multidisciplinary team.
  • Serves as an integral member of the PSM Service Line and may be assigned to GAP coverage or PACT teamlet based on clinical need and coverage.
  • Manages and provides clinical care to a panel of patients under the Primary Care Management Model (PCMM) guidelines.

Requirements

  • Citizenship: non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
  • All applicants tentatively selected for VA employment in a testing designated position are subject to urinalysis to screen for illegal drug use prior to appointment.
  • Applicants who refuse to be tested will be denied employment with VA.
  • English Language Proficiency.
  • In accordance with 38 U.S.C. 7402(f), no person shall serve in direct patient care positions unless they are proficient in basic written and spoken English.
  • Selective Service Registration is required for males born after 12/31/1959.
  • Subject to background/security investigation.

  • Acceptable form(s) of identification will be required to complete pre-employment requirements (https://www.uscis.gov/i-9-central/form-i-9-acceptable-documents).
  • Effective May 7, 2025, driver's licenses or state-issued dentification cards that are not REAL ID compliant cannot be utilized as an acceptable form of identification for employment.
  • Must pass pre-employment physical examination.
  • Participation in the seasonal influenza vaccination program is a requirement for all Department of Veterans Affairs Health Care Personnel (HCP).
