About the Role
This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific eligibility requirements per VHA policy and submit your EDRP application within four months of appointment. Program Approval, award amount (up to $200,000) & eligibility period (one to five years) determined by the VHA Education Loan Repayment Services program office after review of the EDRP application. Former EDRP participants ineligible to apply.
What You'll Do
- →Duties include but not limited to: Conduct comprehensive assessments and in-depth clinical evaluations of patients presenting with cardiovascular symptoms.
- →Conduct comprehensive, complex assessments and detailed clinical evaluations of patients presenting with cardiovascular symptoms, integrating advanced diagnostic reasoning.
- →Order and interpret a full range of advanced diagnostic tests, including EKGs, echocardiograms (including stress testing), Holter monitors, and cardiac catheterizations, ensuring accuracy and comprehensiveness.
- →Lead the management of a diverse panel of patients with both acute and chronic cardiovascular conditions, applying evidence-based practices to promote health and prevent disease.
- →Administer therapeutic interventions independently and monitor patient outcomes, making evidence-based adjustments to treatment plans as necessary.
- →Collaborate extensively with cardiologists and multidisciplinary healthcare providers to develop and coordinate comprehensive patient care pathways, ensuring continuity and quality of care.
- →Provide advanced patient education and counseling on cardiovascular health, risk factors, and tailored lifestyle modifications to empower patients in their self management.
- →Maintain precise and timely documentation of all patient interactions, diagnoses, treatment plans, and outcomes in the electronic health record (EHRM) system.
- →Actively participate in and sometimes lead interdisciplinary team meetings to devise patient care strategies, contribute to case discussions, and drive quality improvement initiatives.

Requirements
