The Physical Therapist (PT) is a member of the Canandaigua VA Medical Center Home Based Primary Care (HBPC) and Physical Medicine and Rehabilitation Teams. The incumbent provides in-home evaluations for the enrolled HBPC patient population, and plans, recommends, and coordinates other pertinent VA or rehabilitation services in the home. The PT also assesses patients for appropriate in-home equipment and provides health education and training to patients and their caregivers.
What You'll Do
- →VA Careers - Physical Therapy: https://youtube.com/embed/Gi2hoFqIoqY Duties include but are not limited to the following: Evaluates patients' functional, physical, and/or psycho-social status to establish a baseline level of functioning.
- →Assesses goals, living skills, physical abilities, caregiver influence on performance, and related needs at admission and as referred.
- →Administers functional outcome measures, including manual muscle testing, goniometry, girth measurements, and ADL/IADL assessments.
- →Maintains professional knowledge appropriate to the age, sex, and ethnic background of patients served.
- →Attends continuing education to maintain licensure and improve job-related knowledge and skills.
- →Completes all required VHA training on an ongoing basis.
- →Attends and participates in HBPC and PMRS staff meetings.
- →Participates in performance improvement and quality monitoring activities.
- →Performs other duties and special assignments as requested by supervisor.

Requirements
