About the Role

Responsible and accountable for all elements of the nursing process when providing and/or supervising direct patient care. Assesses, plans, implements, and evaluates the effectiveness of the Veteran's care. Supports delivery of high-quality, timely care to Veterans receiving services in the community, with a focus on patient education, self-management, and customer satisfaction throughout the continuum of care.

What You'll Do

  • Duties The Community Care (CC) Nurse Case Manager (NCM) is responsible for executing a streamlined approach to receiving, triaging, and directing timely, coordinated care.
  • Key duties include: Clinical Practice & Decision-Making Demonstrates proficient practice and decision-making, deliberate planning, and critical thinking skills.
  • Assesses, plans, implements, and evaluates the effectiveness of each Veteran's care using the full nursing process.
  • Evaluates outcomes of evidence-based decisions and practice changes for individuals, groups, and populations.
  • Follows procedures per established policies and guidelines.
  • Community Care Knowledge & Coordination Possesses in-depth knowledge of the internal referral care process, clinical review criteria, utilization management standards, clinical documentation requirements, community care regulations, and Veterans Health Administration (VHA) standards and guidelines.
  • Uses nursing knowledge and systems theory to assess, organize, facilitate, and guide Veterans through their full range of care options, including internal/direct care in Veterans Affairs (VA) and care in the community.
  • Serves as a liaison to both internal and community providers, managing the care of the Veteran throughout the consult process.
  • Care Coordination & IVC Collaboration Works collaboratively within the Office of Integrated Veteran Care (IVC) to triage Requests for Services (RFS) and coordinate care processes as defined by the IVC Field Guidebook.
  • Develops care coordination plans, reviews medical documentation from community providers, communicates care coordination needs, and supports the Veteran through the completion of the episode of care.
  • Assumes responsibility for the coordination of care focused on patient education, self-management, and customer satisfaction throughout the continuum of care.
  • Interdisciplinary Collaboration & Outcomes Influences care outcomes by collaborating with members of the interdisciplinary team both within the VA and in the community.
  • Supports the medical center's mission to improve timely access to care, ensure Veterans who want to receive care in the community are referred and scheduled appropriately, and support the delivery of high-quality, timely care.
  • Responsible for documented outcomes at the program or service level; outcomes must be broad and complex and can be demonstrated at any organizational level within a facility.

Requirements

  • Citizenship; non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
  • All applicants tentatively selected for VA employment in a testing designated position are subject to urinalysis to screen for illegal drug use prior to appointment.
  • Applicants who refuse to be tested will be denied employment with VA.
  • Selective Service Registration is required for males born after 12/31/1959.
  • Subject to background/security investigation.

  • Must pass pre-employment physical examination.
  • Participation in the seasonal influenza vaccination program is a requirement for all Department of Veterans Affairs Health Care Personnel (HCP).
