About the Role
Join the Indian Health Service at the Wewoka Service Unit and care for diverse American Indian and Alaska Native communities. This position functions in a case management role, providing care to patients across the continuum and collaborates with multidisciplinary health care professionals and community agencies. What You'll Do
- →Total Compensation Package - Check out IHS's outstanding total compensation package for this job: Registered Nurse Total Compensation | Pay (ihs.gov) Manages care of patients through the health care system based on the individual's needs.
- →Coordinates services, health promotion and counseling, disease prevention, health education and screening, and community resources linkage for patients.
- →Applies clinical criteria to monitor and facilitate in house consultations and Purchase Referred Care (PRC) to specialists outside of the facility.
- →Manages aspects of the planning process in anticipation of patient variances or transitions.
- →Develops an individualized comprehensive plan of care in collaboration with physicians and multidisciplinary team.
Requirements
