About the Role
This Licensed Practical Nurse position is located at the York VA Community Clinic, York, PA.The primary purpose of this position is to perform a very wide range of patient personal care, support diagnostic services, and chart activities on a regular and continual basis on a surgical, medical, rehabilitation, psychiatric, or long-term care ward or clinic. The incumbent performs duties in such a way as to promote a sense of worth and dignity within each individual.
What You'll Do
- →The primary purpose of this position is to perform a very wide range of patient personal care, support diagnostic services, and chart activities on a regular and continual basis on a surgical, medical, rehabilitation, psychiatric, or long-term care ward or clinic.
- →The incumbent performs duties in such a way as to promote a sense of worth and dignity within each individual.
- →Support diagnostic procedures - Performing support duties for complex diagnostic test and or specialized practices or procedures.
- →Collaborating with members of other services regarding changes in patient's condition, laboratory results, responses to treatment plan, and other pertinent parameters of care.
- →Participates in Patient Aligned Care Team (PACT) huddles with provider, PACT RN, and clerk on regular basis.
- →Keeps provider on schedule with clinic appointments. - Adhere to ethical and principles and standards in delivering care - The nurse promotes, advocates for, and strives to protect the health, safety, and rights of the patient.
- →Protects all patient information by securing documents and protecting computer screens from the view of others.
- →Practices with respect for autonomy of the Veteran, exercising fairness, truthfulness, and justice.
- →Enter information into patients' records/charts - Triage notes include vital signs, reminders, and patient input/responses.
- →Follows process to ensure that notes from other disciplines, outside providers, and hospitals, are scanned into the patient's record. *Additional duties as assigned.

- →When setting pay, a higher step rate of the appropriate grade may be determined after consideration of higher or unique qualifications or special needs of the VA (Above Minimum Rate of the Grade).
- →Paid Time Off: 37-50 days of annual paid time offer per year (13-26 days of annual leave, 13 days of sick leave, 11 paid Federal holidays per year) Selected applicants may qualify for credit toward annual leave accrual, based on prior [work experience] or military service experience.
- →Parental Leave: After 12 months of employment, up to 12 weeks of paid parental leave in connection with the birth, adoption, or foster care placement of a child.
