The primary purpose of the Licensed Practical/License Vocational Nurse (LPN/LVN) is to perform assigned duties in a manner that promotes quality health care, positive customer service and the maintenance of an environment committed to serving the Veteran. The Licensed Practical/License Vocational Nurse (LPN/LVN) will perform under frequent guidance of the Nurse Manager or designee. Assigned tasks require basic technical competence and skill.
What You'll Do
- →The incumbent will be working at The Huntington Hershel "Woody" Williams VA Medical Center/Nursing Service/PACT facility.
- →The LPN/LVN is responsible and accountable for individual nursing practice.
- →The Patient Aligned Care Team (PACT) Licensed Practical Nurse (LPN) provides direct support to the PACT teams in Ambulatory Care Outpatient Clinics.
- →The incumbent is accountable for greeting the Veterans, using different modalities of gathering Veteran data such as observation, vital signs, and recording information the Veteran provides them; communicating this data to the Registered Nurse (RN) and/or provider; and taking action as directed.
- →They possess basic LPN skills and functions of a new or recently graduated LPN at a GS-3 level, having possess basic LPN skills with at least six months prior experience as an LPN at a GS-4 level , possess a higher level than basic LPN skills with at least one year prior experience as an LPN at a GS-5 level in a direct patient care environment.
- →They assist in assuring smooth clinic flow by assisting the Veteran through the transition process as they move through the clinic environment.
- →The LPN works under the direction of the Nurse Manager, Assistant Nurse Manager, or the RN Care Manager.
- →The LPN/LVN GS-6 is expected to function at a fully successful level for all key elements at the GS-5 grade.
- →The LPN/LVN GS-6 is a full performance functional level for the LPN/LVN occupation for providing practical nursing care with a variety of physical and/or behavioral problems.
