About the Role
This position is located at the Akron VA Clinic Community Based Outpatient Clinic in Akron, OH.
What You'll Do
- →Collaborates with the Home Based Primary Care (HBPC) Interdisciplinary Team including the Physician Preceptor, Registered Nurse, Licensed Practical Nurse, Occupational Therapist/Physical Therapist, Social Work, Dietitian, Speech Therapist, Psychologist, Pharmacist and Advanced Practice Provider.
- →Collaborates with services internal and external to the VA to manage the care needs of the patients.
- →Provides patient and family health education with a focus on self-management, prevention, health promotion and wellness based on the patient's individual goals.
- →Supports additional teams by completing home visits as needed based on patient care demands.
- →Demonstrates strong prioritization skills in determining the order of patient visits according to acuity and need.
- →Utilizes Home Based Primary Care (HBPC) policies and procedures to provide autonomous, high-quality care in the home setting.
- →Possesses the knowledge and skills to carry out more specialized patient care activities.
- →Knowledge of program regulations and is able to carry out objectives of the Nursing program.
- →Contributes to the development of the plan of care for assigned veterans outlining program modalities and updates per policy and as changes occur.
