About the Role

The primary purpose of the Licensed Practical Nurse (LPN) GS-6 is to perform assigned duties in a manner that promotes quality health care, positive customer service and the maintenance of an environment committed to serving the veteran.   • VA Careers - Licensed Practical Nurse: https://youtube.com/embed/Ae85IP1Oiz4 Total Rewards of a Allied Health Professional Major Duties: The Primary Care LPN has knowledge and skills required to perform increasingly complex nursing functions/care including comprehensive primary care, medication management of chronic diseases, and education on health promotion.
  • Provisions of Care, including but not limited to: Works with Primary Care team/leadership to improve access to care, including clinical provider support and veteran education.
  • Collaborates with Primary Care team/leadership to communicate and coordinate with PACT teams to develop and implement treatment plans for veterans.
  • Provides support to the Primary Care teams (all) when needed.
  • Perform assigned duties in a manner that promotes quality healthcare, positive customer service, and the maintenance of an environment committed to serving the veteran.
  • Oxygen administration if applicable Uses the nursing process and evidence-based practice to collaborate with the RN Care Manager, Clinical Pharmacy Specialists, along with various Primary Care and interdisciplinary team members to develop veteran driven holistic care.
  • Medication administration to include Immunizations.
  • Clinical reminders with appropriate consults and alerts as indicated.
  • Check expiration dates of medications and supplies Delegated procedures and treatments.
  • Responsible for observing, recording, and reporting changes in the veteran's conditions, as well as assisting in the coordination of care that includes the veteran's preferences.
  • Assists with minor procedures, including obtaining and arranging necessary equipment, positioning patient, ensuring comfort and privacy.
  • Monitors patient during the procedure.
  • Completes mandatory reviews, including motivational interviewing, in order to promote patient self-management toward individualized life plan of care.
  • Conduct chart audits as assigned.
