The Licensed Practical Nurse (LPN) - Patient Aligned Care Team (PACT) will perform assigned duties in a manner that promotes quality health care, positive customer service and the maintenance of an environment committed to serving the veteran. The LPN will also provide safe and individualized nursing care to patients in the Primary Care setting. The incumbent will work at our Beckley VA Medical Center located in Beckley, West Virginia (Raleigh County).
What You'll Do
- →Below are some of the primary duties and responsibilities of a Patient Aligned Care Team (PACT) Licensed Practical Nurse (LPN: Prepares and administers prescribed medications via (oral, topical, subcutaneous, intramuscular and/or intravenous) and performs treatments according to established policies and procedures.
- →Takes appropriate actions based upon communications, observations, veteran care reports, provider/staff discussion.
- →Assists the RN/provider and other staff in proactively identifying and addressing veteran care needs.
- →Accurately and timely records all patient data and education into the medical record.
- →Knowledge and skill in performing support duties for diagnostic tests and / other specialized equipment and monitoring the Veteran's condition before, during , and following the procedure.
- →Work Schedule: Monday-Friday, 8:00am-4:30pm Telework: Not Available Virtual: This is not a virtual position.

Requirements
