About the Role
This position is located at the Richard L. Roudebush, IN VA Clinic within the Primary Care PACT Service. The Licensed Practical Nurse works under the direction of an RN and reports to the primary care manager. The LPN is responsible for observing, recording and reporting changes in the patient's condition, as well as assisting in the development and revision of the patient-driven holistic care plan for life.
What You'll Do
- →Total Rewards of a Allied Health Professional SCOPE OF DUTIES: Scope of Duties:The Licensed Practical Nurse works under the direction of an RN and reports to the primary care manager.
- →The LPN is responsible for observing, recording, and reporting changes in the patient's condition, as well as assisting in the development and revision of the patient-driven holistic care plan for life.
- →The LPN is a member of the larger Team.
- →The LPN at the GS-6 grade level perform all duties expected of employees at the GS-5 grade level.
- →The LPN will have the Knowledge and ability to appropriately carry out assigned patient care based on the patients' conditions; to use judgment in selecting the appropriate order and sequence of procedures and treatments; and to accurately recognize, report and record relevant patient information.
- →Completed work should need only a general review by a RN or MD/DO for appropriateness and conformity with established policies/procedures.
- →The LPN will work under the general supervision of a RN or MD/DO.
- →The LPN demonstrates active collaboration and communication with Teamlet (Patient, Primary Care Provider, RN Care Manager and Clinical Support Technician) and Larger Team (family/caregiver, internal and community-based services and disciplines involved in providing care to the patient).
- →Work Schedule: Monday - Friday 07:30 - 16:00 Compressed/Flexible: Available at supervisor discretion.
- →Recruitment Incentive (Sign-on Bonus): Not Authorized Permanent Change of Station (Relocation Assistance): Not Authorized Pay: Competitive salary and regular salary increases.
- →When setting pay, a higher step rate of the appropriate grade may be determined after consideration of higher or unique qualifications or special needs of the VA (Above Minimum Rate of the Grade).
- →Paid Time Off: 37-50 days of annual paid time offer per year (13-26 days of annual leave, 13 days of sick leave, 11 paid Federal holidays per year) Selected applicants may qualify for credit toward annual leave accrual, based on prior [work experience] or military service experience.
- →Parental Leave: After 12 months of employment, up to 12 weeks of paid parental leave in connection with the birth, adoption, or foster care placement of a child.
- →Child Care Subsidy: After 60 days of employment, full time employees with a total family income below $144,000 may be eligible for a childcare subsidy up to 25% of total eligible childcare costs for eligible children up to the monthly maximum of $416.66.
