About the Role

This Licensed Practical Nurse (LPN) is directly responsible to the Registered Nurse and/or preceptor for providing direct care within his/her scope of practice. The Nurse Manager/designee is responsible for the supervision and performance evaluation.

  • The CLC supports 51 veterans for rehabilitation, hospice and continuing care needs.
  • This position is essential to complete Advancing Excellence software package requirements.
  • Consistent data collection improves the quality of care and quality of life for residents cared for within the Community Living Center.
  • The LPN can also complete medication passes, observations of residents, restorative care for residents, and data collection for LTC standards.
  • The LPN will provide data for the assessment under the direction of the RN.
  • The LPN will provide input into the interdisciplinary planning of care with development of resident-centered goals and outcomes with a rehabilitative/restorative/support focus.
  • The LPN will observe the resident's condition and progress toward the plan of care, report unexpected changes to the appropriate team members in a timely manner and document appropriately.
  • The LPN will implement the plan of care with focus to prevent deterioration, achieve and maintain optimal levels of function/independence and promote comfort.
  • The LPN will provide rehabilitative/restorative/supportive nursing care daily, which is directed toward measures to prevent deterioration and to achieve or maintain optimal levels of function, independence and comfort.
  • The LPN will recognize population-specific resident needs and will apply his/her knowledge in delivering care.
  • Pay Scale: V744 Work Schedule: Days/Nights 12 hours with rotation to other shifts as needed.
  • Recruitment/Relocation Incentives (Sign-on Bonus): Not authorized Permanent Change of Station (Relocation Assistance): Not authorized Pay: Competitive salary and regular salary increases.
  • When setting pay, a higher step rate of the appropriate grade may be determined after consideration of higher or unique qualifications or special needs of the VA (Above Minimum Rate of the Grade).
