This position will provide quality outreach health care services and community-based health promotion and disease prevention services. A REAL ID will be required beginning May 7, 2025, in accordance with 6 C.F.R. 37.5 (2021).
What You'll Do
- →Incumbent will be required to provide medically related transportation for community members focusing on the elderly and adolescents that are recognized with lack of transportation resources.
- →Incumbent's primary responsibilities are to provide home health visits to high risk elderly and adolescent clients in coordination with the Public Health Nursing and or Director of Nursing.
- →Provides education on healthy lifestyle habits to all clients.
- →Notify clients of scheduled medical appointments and follow-up from PHNs, Charge Nurse and referred providers appointments.
Requirements
- ✓Selectee may be subject to a probationary period ESEP appointees typically serve a two year trial period U.S.
- ✓Citizenship is required Selective Service Registration is required for males born after 12/31/1959 Must have unrestricted valid state issued drivers license (any state).
- ✓This is a designated position covered by Public Law 101-630, requiring contact or control over Indian children.

- ✓On-call hours or standby duty may be required.

- ✓Background Investigation Requirement: This position requires the successful completion of a federal background investigation as a condition of employment.

- ✓Upon receipt of instructions, applicants must complete all required background investigation forms within five (5) calendar days of issuance.
