The Connected Care Coordinator reports to the FTC & supports Veteran communication at NAVAHCS by promoting MyHealtheVet, Secure Messaging, and other digital health tools. The role leads training, outreach, implementation, and reporting to improve access, engagement, and coordination across VA's connected care technologies.
What You'll Do
- →Major duties include but are not limited to: Assists with the troubleshooting, testing and reporting of problems with all Connected Care and VHRC operations.
- →Maintains knowledge in how to set-up and maintain multiple Connected Care health programs, as needed to test and troubleshoot programs.
- →Maintains knowledge of a variety of personal technology devices and how they interface with Connected Care Health programs.
- →Provides leadership involving workgroup transition issues.
- →Evaluates and makes recommendations for effective facility changes.
- →Manages multi-disciplinary teams comprised of professional, technical and support personnel to coordinate clinical and business functions for the Connected Care program and VHRC.
- →Works with senior functional subject matter experts to identify and manage significant changes in the way employees will integrate Connected Care Health in the fabric of VA care and the Patient-Centered Medical Home.
- →Trains clinic support staff on use of the Connected Care applications.
- →Develops, implements, maintains, and documents Connected Care internal and external control policies and procedures.
- →Formulates a marketing strategy from research, reports, products and organizational data.
- →Designs, conducts, and evaluates innovative means to advertise and market products of Connected Care to include creating content for local NA V AHCS social media accounts.
- →Leads innovations to keep facilities Connected Care cutting-edge.
- →Ensures accountability in written communications as it relates to Connected Care as these communication skills contributes importantly to the way VHA communications with Veterans, Congress, our dedicates volunteers, the public and employees.
- →Maintains liaison with the VISN and National Connected Care teams and coordinators to ensure correct, current and accurate portrayal of information is provided to meet objectives.
