About the Role
This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific eligibility requirements per VHA policy and submit your EDRP application within four months of appointment. Program Approval, award amount (up to $200,000) & eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after review of the EDRP application. Former EDRP participants ineligible to apply.
What You'll Do
- →VA Careers - Pharmacy: https://youtube.com/embed/Fn_ickNBEws The Clinical Pharmacist Practitioner (Anticoagulation Clinic) is a licensed pharmacist having completed a pharmacy practice or specialty residency or having other equivalent relevant clinical experience.
- →The incumbent functions under a scope of practice and is primarily responsible for clinical pharmacy management of the medical center Anticoagulation Clinics.
- →The majority of the incumbent's time will be spent providing comprehensive pharmaceutical care to patients in this practice area.
- →Additional functions may include clinical pharmacy support of various Primary Care clinics.
- →Duties include and are not limited to: Clinical: 1.
- →Develops and maintains an in-depth knowledge of antithrombotic/anticoagulation management including pharmacotherapy, laboratory monitoring, and relevant medico legal aspects of anticoagulation management. 2.
- →Manages the global and daily administration of the medical center Anticoagulation Clinic including clinic consults, home anticoagulation consults, database management, laboratory compliance of point-of-care monitors, individual clinic patient anticoagulation management, quality review, and clinic personnel training. 3.
- →Develops professional relationships with non-pharmacy professional personnel, and projects the highest level of pharmacy practice and competence. 4.
- →Monitors patient therapy utilizing advanced skills in physical assessment, pharmaceutics, pharmacokinetics, pharmacodynamics, and interpretation of laboratory and other diagnostic parameters. 5.
- →Reviews and evaluates prescribed drug therapy daily for appropriateness, effectiveness, cost-benefit, adverse drug reactions, allergies, drug-drug and drug-disease interactions, and communicates and documents recommendations to providers and pharmacists as necessary. 6.
- →Appropriately renders pharmaceutical care to Anticoagulation Clinic and Primary Care patients and documents care in the medical record. 7.
- →Designs and performs relevant drug use evaluations under direction of the Associate Chief of Pharmacy, Clinical & Educational Services.
- →Participate in research studies in ambulatory care area as assigned Distributive: 1.
- →All inpatient/outpatient medication orders are checked to include patient information, correct medication, dose, route, and frequency of administration.
