About the Role
The Primary Care Licensed Vocational Nurse (LVN) provides high-quality, evidence-based care for a culturally diverse Veteran population; emphasizing patient centered care to veteran's ages 18 to 75 and above in the outpatient care setting. They must be able to promote a safe environment for all patients including those with altered sensorium due to medication or an acute or chronic illness.
What You'll Do
- →The Licensed Vocational Nurse performs assignments in the Community Based Outpatient Clinic in the Rancho Cucamonga , CA location as a member of the Patient Aligned Care Team (PACT).
- →Under the direction of the Registered Nurse or Nurse Manager, she/he is responsible for providing direct and/or indirect patient care to an assigned group of patients.
- →This care takes into consideration all physical, psychosocial, spiritual and cultural needs of this specific Veteran population.
- →Preferred Experience: 2 year experience PACT experience 2 years out-patient clinic experience Procedure experience LVN skilled nursing experience Work Schedule: TBD Telework: Not available.
