About the Role
The Hampton VAMC is recruiting for a Vascular Access Nurse (Dialysis) within the SCI and Rehab Department. VA Nurses are integrally involved in the greater federal, national, and international communities of nursing: advocating for informed nursing practice so we can constantly improve nursing care for our nation's heroes with our nursing colleagues.
What You'll Do
- →The Nurse I, Level 1 Vascular Access Registered Nurse (RN) is responsible and accountable for individual nursing practice and determines the appropriate delegation of tasks consistent with the nurse's obligation to provide optimal care.
- →The Nurse II Vascular Access Registered Nurse (RN) is responsible for providing competent, evidence-based care to assigned patients and oversight of licensed vocational nurses/nursing assistants, as appropriate to the setting.
- →The Nurse III Vascular Access Registered Nurse (RN) executes position responsibilities that demonstrate leadership, experience, and creative approaches to management of complex patient care.
- →Responsible for working in close collaboration with the Nurse Manager, Chief of Nephrology, General Vascular Surgery and community vascular access resources to ensure and provide program services to patients requiring vascular access who have acute and chronic renal failure.
- →Provides direct patient care to peritoneal dialysis Veterans and ongoing support, oversight and education to teammates, provider partners, and hemodialysis patients in the care and management of vascular accesses.
- →Ensures that the appropriate plan of care is implemented according to the standards of regulatory agencies, such as JC, OSHA and VHA Directives.
- →Proactively collects and reports accurate and complete data and assessments of the accesses to facility, teammates, patients and other care providers for baseline and ongoing outcomes Maintains access history and ensures documentation of care and outcome and communicates vascular access-related outcomes and status.
- →Core elements of performance include knowledge and active participation in unit/program level quality improvement processes and initiatives as well as customer service programs.
- →Possesses the knowledge and skills to effectively apply all aspects of the nursing process within a collaborative, interdisciplinary practice setting to facilitate the effective care of Veterans.
- →Monitors vascular access and hemodialysis care delivery identifying access dysfunction and opportunities to increase arterio-venous fistulas (AVFs) and reduce catheters.
- →Facilitates the process of referral for diagnosis of those accesses with dysfunction or other pathology.
- →Provides direct patient care in the dialysis unit or ICU coverage for hemodialysis therapy for safe patient staffing and may be required to provide backup coverage for the PD Nurse Coordinator if he/she is not available for education and training of patient and/or family.
- →Participates in daily huddles providing Dialysis Quality Assurance meeting to provide optimal care for patients receiving chronic outpatient dialysis therapy.
- →Maintains clinical on-call requirements for RDU nursing staff.
Requirements

- ✓Participation in the seasonal influenza vaccination program is a requirement for all Department of Veterans Affairs Health Care Personnel (HCP).
