About the Role

The Supervisory Social Worker for the Community Resource and Referral Center is licensed at the Advanced practice Level and functions as the Supervisory staff for the Community Resource and Referral Center (CRRC) and all the project-based programs assigned to the City of Chicago and the catchment area in Indiana for Jesse Brown V A Medical Center located 820 S. Damen Ave, Chicago, IL.

What You'll Do

  • The Supervisory Social Worker -Community Resource and Referral Center -Health Care for Homeless Veterans (HCHV) is responsible for supervising staff based out of Jesse Brown VAMC.
  • Incumbent demonstrates possession of advanced practice skills and progressively demonstrates more professional competency and judgment.
  • The incumbent is responsible for professional and administrative management of the Outreach Program and serves monitors, and overseas to ensure that patient eligibility, enrollment, and care is coordinated with the VA Medical Center, as well as community health agencies and mental health providers that serve the homeless populations.
  • The incumbent is responsible for managing the Homeless Outreach Teams by overseeing service delivery to Veterans and is responsible for building and maintaining relationships with community agencies, recruiting Outreach Team staff members, organizing, and attending community meetings and event.
  • Major Duties Three years of specialized practice in a hospital or clinic program providing treatment to patients in a major specialty, such as Healthcare for Homeless Veterans Program (HCHV) or other specialized health or mental health programs are preferred.
  • The incumbent develops work improvement plans and recommends personnel actions as appropriate to the maintenance of the program.
  • The incumbent possesses the skills to offer psychosocial interventions and measures outcomes to interventions.
  • The incumbent provides advice and counsel related to work and administrative matters and implements disciplinary measures after consulting with Labor Relations and Chief of Social Work Service as needed.
  • The CRRC Supervisory Social Worker possesses skills to assist teammates with clinical consultation and oversight for programmatic concerns.
  • The incumbent supervises employees and exercises supervisory authority regarding responsibilities involving work assignments and performance evaluations; and selection of staff, training, and recommendation of awards, advancements, and when appropriate, disciplinary action.
  • The Supervisory Social Worker for the CRRC is responsible for requesting and appropriating the annual program budget, as appropriate, including Finds control, contracts, and equipment and is responsible for establishing Homeless Outreach goals, objectives, and performance measures.
  • The incumbent is responsible for inputting monthly data into all needed reports requested by the VISN and National Offices.
