About the Role

This position serves in a combined clinical and supervisory role for the PACT-CBOC and Medical Foster Home (MFH) programs. Services are provided at the assigned Community Based Outpatient Clinic/s aligned with the Dorn VA Medical Center in Columbia, SC, as well as in the community.

  • Independently complete thorough SDOH assessments to identify needs of Veterans and Family members.
  • Actively participate as a member of the interdisciplinary treatment team, collaborating with Veterans, families, and team members to develop and implement treatment goals and interventions.
  • Facilitate community placements including referrals to group and family care homes, assisted living facilities, adult health care programs, and nursing homes.
  • Independently implementing treatment modalities and evidence-based practices to provide education to Veterans and/or family members.
  • Provide consultation and education to Veterans/family regarding community resources, VA benefits, specialty programs, and Advance Directives.
  • Provide consultation to treatment team and staff members regarding Veterans' and families' social needs and the impact of SDOH on health care planning and treatment compliance.
  • Understanding caregiver stressors, conducting caregiver assessments, and providing appropriate caregiver-specific interventions and support.
  • Providing administrative, technical, and clinical supervision of assigned CBOC PACT social work staff, including: Setting priorities and assignments.
  • Setting performance standards and reviewing work.
  • Reporting workload data to the Chief, SWS, PACT team leader, and/or the Chief of the CBOC.
  • Observing performance and providing instruction, technical advice, counseling, informal feedback, and periodic evaluations.
  • Developing work improvement plans and recommending personnel actions.
  • Conducting Focused Professional Practice Evaluations (FPPE) and Ongoing Professional Practice Evaluations (OPPE).
