About the Role
The Supervisory Social Worker functions as a member of the Homeless Patient Aligned Care Team. The incumbent will focus on housing issues, securing housing, using available homeless resources, prevention of homelessness, health promotion, connecting and coordination of multiple services related to homeless Veterans care needs. These positions provide support to the National Center for Warrior Independence at the VA Greater Los Angeles Healthcare System in Los Angeles, California.

- →VA Careers - Social Work: https://youtube.com/embed/enRhz_ua_UU This announcement will fill Senior Social Worker vacancies within the Community Engagement and Reintegration Service (CERS) located at the Greater Los Angeles Healthcare System in Los Angeles, California.
- →Duties may include but are not limited to: Assesses and treats the complicated psychosocial problems of Veterans, families, and significant other.
- →Provides assessment of homeless and other psychosocial problems that cause distress, can include adjustment to the current medical condition and/or functional status, untreated or under- treated mental health or substance abuse condition, economic instability, legal problems, and inadequate social supports, housing and transportation.
- →Participate in improving chronic disease outcomes by providing primary and secondary preventive care services and self-management tailored to homeless Veteran needs.
- →Demonstrates sound knowledge and understanding of substance abuse, chronic mental illness, and issues of chronic homelessness and isolation.
- →Develops and maintains positive working relationships with homeless patient's case managers and communicate with them to work together to support patients in obtaining and maintaining permanent housing.
- →Conducts comprehensive assessments of Veterans and develops treatment plans that are sensitive to Veterans' diverse needs and abilities.
- →Independently performs a full range of clinical services including Individual, Group and Family interventions.
- →These services are supported through team meetings, chart documentation, meetings with family and collaterals as other agencies (i.e.
- →SS, VA hospitals, Community Based Outpatient Clinic's, private agencies etc.) to maximize the successful stabilization and integration of Veterans in the community.
- →Develops and implements methods for measuring effectiveness of social work practice and services in specialty areas, utilizing outcome evaluations to improve treatment services.
- →Work Schedule: Various tours of duty based on the needs of the service.
- →Compressed/Flexible: May be available based on the needs of the service.

Requirements
- ✓You must be a U.S.

- ✓Selective Service Registration is required for males born after 12/31/1959.
