About the Role

The Residential Social Worker is responsible for individual and group therapy and case management for Veteran patients on one of three VAMHCS residential programs, but primarily the Domiciliary. The Perry Point residential treatment programs include a homeless Domiciliary, a Psychosocial Rehabilitation Residential Treatment Program (PRRTP), and a Substance Abuse Rehabilitation Residential Treatment Program (SARRTP).

What You'll Do

  • DUTIES: The Residential Social Worker is responsible for individual and group therapy and case management for Veteran patients on one of three VAMHCS residential programs, but primarily the Domiciliary.
  • The Perry Point residential treatment programs include a homeless Domiciliary, a Psychosocial Rehabilitation Residential Treatment Program (PRRTP), and a Substance Abuse Rehabilitation Residential Treatment Program (SARRTP).
  • Incumbent performs specialized treatment of complex physical and/or mental illness, incorporates complex multiple causation in differential diagnosis and treatment of veteran patients, including making psychosocial and psychiatric diagnosis within approved boundaries of clinical privileges and scope of practice.
  • Training and proficiency in Evidence Based Psychotherapies relevant to patients living with Mental Health and Substance Use Disorders; facilitates individual and group therapy.
  • Use of evidence based tools to measure treatment effectiveness and guide treatment Incumbent provides case management services, which may include referrals for housing, transportation, and/or food, to assist unhoused and/or unemployed person's in improving long term functioning and outcomes.
  • Assists with local, State and Federal benefits and assists with applications as necessary.
  • Schedules discharge appointments and coordinates transportation.
  • Incumbent will network both internally and externally to the facility and coordinate community-based services as may be necessary; this may include information and referral for additional services from other VA programs, other government programs and community agency programs.
  • Social Worker may transport Veteran patient in the community for discharge planning purposes.
  • Establishes and maintains effective therapeutic relationships with Veterans and their families.
  • Incumbent will complete appropriate suicide screens, assessments, and safety plans.
  • Work Schedule: Monday - Friday. 8:00am-4:30pm Telework: ADHOC ONLY Virtual: This is not a virtual position.

  • You must be a U.S.
