About the Role

The Samuel Stratton VA Medical Center, located in Albany, NY is seeking a qualified Social Worker to join its team.

What You'll Do

  • This position is located in the Social Work Service at the Stratton VA Medical Center in Albany, New York.
  • The incumbent is assigned to the Homebased Primary Care Program (HBPC) and must use a high level of skill in assessing and treating the complicated psychosocial problems of Veterans.
  • Duties include, but are not limited to: Social Work responsibilities include the assessment of psychosocial problems, often at a time when the veteran is experiencing crisis and include the provision of short-term, solution-focused counseling with veterans and/or their family members/significant others.
  • Referrals typically originate from members of the interdisciplinary team, from veterans and/or their family members/significant others, and from community professionals.
  • The incumbent must complete thorough assessments to determine the presenting problem to determine the factors impacting the problem and to determine obstacles to treatment and/or follow through with recommended treatments.
  • Consultation with members of the interdisciplinary team is provided in order to coordinate a collaborative effort to meet the agreed upon goals for the veteran's treatment needs and to include liaison with community professionals.
  • The majority of patients are elderly or disabled Veterans with complex, chronic medical and psychosocial needs who present with issues related to continuity of care, care coordination as well as emergent situations.
  • Social Work service is provided in the patients' homes as well as in community settings such as community nursing homes and Assisted Living Facilities.
  • Extensive driving is required, operating a GSA vehicle per policy.
  • This position will also provide coverage to social work walk -in clinic and social work consults.
  • The incumbent may be required to work an irregular tour of duty and weekends, depending on the needs of the service, typically on a rotating basis, as well as participate in the On-Call social work coverage rotation.
  • This position requires travel to patients' homes utilizing government vehicles; valid drivers license is required.
  • Paid Time Off: 37-50 days of annual paid time offer per year (13-26 days of annual leave, 13 days of sick leave, 11 paid Federal holidays per year) Selected applicants may qualify for credit toward annual leave accrual, based on prior [work experience] or military service experience.
  • Parental Leave: After 12 months of employment, up to 12 weeks of paid parental leave in connection with the birth, adoption, or foster care placement of a child.
