About the Role
This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific individual eligibility requirements in accordance with VHA policy and submit your EDRP application within four months of appointment. Program Approval, award amount (up to $200,000) and eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after complete review of the EDRP application.

- →VA Careers - Licensed Clinical Social Worker: https://youtube.com/embed/U_xC25QsN0w Total Rewards of a Allied Health Professional The incumbent is assigned as a Social Worker and the Substance Use Disorder (SUD) specialist in Primary Care Mental Health Integration (PC-MHI), who works at the interface between primary care and mental health care in an integrated care model.
- →Duties: The incumbent routinely provides curbside consultations with Primary Care Providers (PCP) and other PACT team members about plan of care for patients and provides feedback in a timely manner.
- →The incumbent is able to function as a BHS or Care Manager in the BHL to assist in the delivery of evidence-based care for primary care patients identified with a variety of emotional, psychiatric, and SUD problems.
- →The incumbent independently performs case finding functions and outreach, identifying and screening patients for psychosocial needs.
- →Participates in scheduling, triaging, and monitoring the flow of patients in the PC-MHI, including BHL.
- →Contributes to interdisciplinary care planning conferences, and team huddles/meetings, especially when SUD are involved.
- →Adheres to and supports operational policies and procedures of the PACT, PC-MHI, MHS and the Medical Center.
- →Assists the program PC-MHI Program Manager in the execution of ongoing program evaluation and data-driven quality improvement projects to monitor the effectiveness of the clinical program.
- →Serves on committees, work groups, and task forces at the facility, VISN, and/or VA Central Office levels.
- →Maintains a level of productivity and quality consistent with facility and Social Work standards and the complexity of the assignment.
- →Participates in appropriate service/facility meetings and Social Work meetings.
- →Other duties as assigned.
- →Work Schedule: Monday-Friday, 8:00am-4:30pm EDRP Authorized: Former EDRP participants ineligible to apply for incentive.
