About the Role
HUD-VASH is an intensive case management program for chronically homeless veterans aimed at providing psychosocial services to veterans while residing in stable housing through HUD's Section 8 voucher system.
What You'll Do
- →The incumbent in this position functions as the Case Manager for VA Central Western Massachusetts' Housing and Urban Development / VA Supportive Housing (HUD-VASH).
- →The HUD-VASH Program falls within the Healthcare for Homeless Veterans (HCHV) Program of the Social Work Service Line.
- →The incumbent is responsible for coordinating wrap-around support services for veterans enrolled in HUD-VASH.
- →Duties include but are not limited to: Performs site visits of HUD-VASH apartments to ensure that veterans reside in safe environments in compliance with local housing codes.
- →Conducts psychosocial assessments to determine needs of veterans and family members.
- →Participates in discharge planning with other disciplines and will be responsible for ensuring that discharge plans are executed in a manner that is timely and appropriate.
- →Provides consultation/education to veterans and families on community resources, advance directives and VA benefits.
- →Coordinates care including serving as an advocate on behalf of the veterans to ensure comprehensive service delivery, and linking and referring veterans to the VA Medical Center, VA Regional Offices and/or community agencies.
- →Other duties as assigned.
- →Work Schedule: Monday - Friday, 8:00 a.m. - 4:30 p.m.

- ✓Selective Service Registration is required for males born after 12/31/1959.
- ✓Must be proficient in written and spoken English.
- ✓Subject to background/security investigation.

- ✓Participation in the seasonal influenza vaccination program is a requirement for all Department of Veterans Affairs Health Care Personnel (HCP).
