About the Role

This position is located within the Medical Social Work Service. The selected applicant will be required to provide Social Work Case Management services in the community to the veteran population enrolled at the John D. Dingell VA Medical Center. Services include inpatient/outpatient service delivery in accordance with the PACT model of care. The Social Worker is also assigned to the SAMS Program, providing initial contact with Veteran family members/caregivers following a Veteran's death.

  • Responsible for developing the treatment plans and setting achievable treatment goals with the veteran/family.
  • The in-patient social worker will work collaboratively with the veteran's assigned Firm/clinic social worker as appropriate for discharge planning, and community services.
  • Responsible for educating the veteran and/or caregiver of the available services and assisting them in establishing the appropriate referrals based on the veteran's preference or that of his surrogate decision-maker.
  • Has knowledge, skills, and abilities of practice related to a special population such as the caregivers and survivors of Veterans who have passed away; particularly those who may not have a known or documented next of kin.
  • The SAMS Social Worker will adhere to the NASW Code of Ethics, all local facility policies, and maintain an unrestricted, unsuspended license to practice social work.
  • Collaborates with interdisciplinary teams throughout the medical center on SAMS Program issues.
  • Demonstrates skill in communication and can develop positive, effective relationships with all SAMS stakeholders including Veterans, survivors, local, state, and federal agencies, as well as non-governmental organizations.
  • Responsible for serving as a consultant for navigating the complex processes survivors encounter when interacting with government and community agencies.
  • Experienced in making rapid assessments and developing crisis management plans for referral to appropriate Mental Health services.
  • Provides education related to VA and community resources, entitlements, advance directives/Living Will and will refer veterans/families to the appropriate interdisciplinary team member for identified health education needs.
  • Work Schedule: Monday-Friday, 8:00AM-4:30PM.
  • Schedule may be subject to change based on the needs of the Agency.

  • Once licensed or certified, social workers must maintain a full, valid, and unrestricted independent license or certification to remain qualified for employment.
  • Loss of licensure or certification will result in removal from the GS-0185 social worker series and may result in termination of employment.
