About the Role

This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific individual eligibility requirements in accordance with VHA policy and submit your EDRP application within four months of appointment. Program Approval, award amount (up to $200,000) and eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after complete review of the EDRP application.

  • VA Careers - Social Work: https://youtube.com/embed/enRhz_ua_UU Total Rewards of a Allied Health Professional Duties include but are not limited to: Responsible for caseload oversight, monitoring and administration, serving as the primary contact with local public Housing Authorities (PHAs).
  • The incumbent will provide case management services necessary to ensure that veteran care is coordinated with the VA medical center, PHAs, community health providers, community mental health providers, and agencies that serve homeless populations.
  • Works directly with veterans within various community housing settings and community sites and proves supportive case management services and partners with community agencies to link access to community-side housing services for veterans.
  • Provides services such as psychosocial assessments, treatment planning, individual, group and family education, and coordination related to these services as clinically indicated.
  • The incumbent will provide case management and referral assistance to the veterans that assist consumers in meeting their goals and objectives within a psychosocial rehabilitation framework that allows successful reintegration into community living.
  • Provides social work services both at field locations and at the medical center, with a supervisor available for consultation.
  • The program requires active interface with the staff of other outpatient and inpatient treatment components as well as networking in the community with landlords, housing programs, PHAs and other housing service providers.
  • The incumbent completes, maintains and assures the accuracy and timeliness of all statistical information entered into HOMES and other data management systems.
  • Provides case management and care coordination to facilitate appropriate delivery of health care services, incorporates complex multiple causations in differential diagnosis and treatment of Veterans, including making psychosocial and psychiatric diagnoses within approved clinical privileges.
  • The incumbent links Veterans with services, resources, and opportunities, to maximize independence, health, and well-being.
  • The incumbent conducts timely assessment of Veterans in crisis to identify immediate needs, evaluate risk, and initiate safety planning as appropriate.
