About the Position: This position is located in a Military Treatment Facility that has received state designation as a Level II Trauma Center. Incumbent works under the general supervision of the Trauma Program Manager (TPM) who defines objectives and changes in policy. This is a Direct Hire Solicitation
What You'll Do
- →Provides professional Social Work interventions to patients and family members affected by traumatic incidents.
- →Provides proactive intervention in discharge planning activities for trauma patients.
- →Consults with other staff, civilian, military resources and community agencies to plan and coordinate patient care throughout the continuum.
- →Participates in development, implementation, and documentation for Quality Assurance.
- →Provides outpatient case management services for designated patient population.
- →Serves as consultant for Civilian Care Coordination.
- →Serves as trauma consultant to staff and providers on psychosocial issues, family dynamics and adjustment to illness.
- →Screens traumatically injured patients for potential drug and alcohol use/abuse.
