About the Role

The Home Based Primary Care (HBPC) Social Worker primary responsibilities are to provide clinical evidence based services and interventions, Veteran and staff education on caregiver issues, community outreach, resource development, continuous quality improvement activities, and evaluation/consultation. The HBPC Social Worker must be a skilled communicator capable of developing effective partnerships with local, state and federal agencies as well as non-governmental organizations.

What You'll Do

  • Plays a key role in the HBPC effort to stabilize these Veterans in their homes and to prevent unnecessary hospitalizations, ER visits, and nursing home stays.
  • Provides psychosocial assessments within 30 days of the patients admission to the program, health screening, care planning, in order to prevent or minimize problems that may interfere with the Veteran's ability to stay in the home.
  • Educates interdisciplinary team members about general and individual psychosocial risk factors and develops and coordinates psychosocial treatment plans with the HBPC team.
  • Provides the Veterans and their caregiver with ongoing clinical counseling.
  • Coordinates with the HBPC team and community agencies to facilitate Veteran compliance with treatment plans to achieve treatment outcomes.
  • Educates and counsels Veteran and family about compliance and consequences of non-compliant behavior and completes behavioral contracting with identified Veterans to promote compliance.
  • Assesses progress toward Veteran, professional, ·and program goals in order to improve performance as evidenced by participation in team meetings, self-assessment activities, and performance improvement activities.
  • Participates with the HBPC team in identifying program outcomes measures and monitors practice for efficiencies, achieved outcomes, and costs.
  • Supports the Home Based Primary Care Program, Hospice/Palliative Care Program and other Geriatric and Extended Care (GEC) programs as needed and may provide Social Work services to Veterans managed in these Geriatrics and Extended Care based programs.
  • Acts as consultant and liaison between VA and community resources.
  • Provides case management to Veterans and families, as needed, throughout the continuum of care.
  • The HBPC Social Worker will provide a full range of Social Work services within commonly accepted standards of Social Work practice.
  • Participates fully in peer review, assessing the quality of professional social work standards provided in the organization.
  • Paid Time Off: 37-50 days of annual paid time offer per year (13-26 days of annual leave, 13 days of sick leave, 11 paid Federal holidays per year) Selected applicants may qualify for credit toward annual leave accrual, based on prior [work experience] or military service experience.
