About the Role

This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific eligibility requirements per VHA policy and submit your EDRP application within four months of appointment. Program Approval, award amount (up to $200,000) & eligibility period (1-5 years) are determined by the VHA Education Loan Repayment Services program office after review of the EDRP application. Former EDRP participants ineligible to apply.

What You'll Do

  • The incumbent in this position functions as a Clinical Case Manager for the Manchester VA Medical Center's HUD/VASH program.
  • Housing and Urban Development/Veterans Affairs Supported Housing (HUD/VASH) is an intensive clinical case management program for chronically homeless Veterans aimed at providing psychosocial services to Veterans while residing in stable housing through HUD's housing choice voucher system.
  • The incumbent is responsible for providing case management for a caseload of veterans in the HUD/VASH program.
  • The incumbent is responsible for reviewing agreements between the veteran, the landlords, and the Public Housing Authorities to assure that the HUD/VASH placement site remains a permanent housing option for veteran.
  • The incumbent must be able to develop and manage complex therapeutic relationships with the Veterans as the service may be provided in varied community, home and VHA sites.
  • The incumbent is expected to utilize Motivational Interviewing and/or other evidence-based practices to meet the Veteran where he or she is in the treatment engagement process.
  • The Social Worker also will need to develop liaisons with treatment providers/programs at the Manchester VAMC and in the community.
  • The Social Worker will provide appropriate group and individual assessment, case management and treatment.
  • The HUD/VASH Social Worker provides direct clinical and case management services to Veterans enrolled in the HUD/VASH program.
  • The incumbent will provide consultative and collaborative services to other Health Care for Homeless Veterans (HCHV) programs, including offering clinical services to Veterans enrolled in other HCHV programs as needed.
  • The HUD/VASH Social Worker will be responsible for maintaining a caseload of HUD/VASH enrolled Veterans.
  • The determination of the size of the caseload will be dependent upon the complexity of the Veterans on the incumbent's caseload and the needs of the HCHV and HUD/VASH programs.
  • The HUD/VASH Social Worker will participate in regularly occurring clinical supervision in order to prepare for advanced practice licensure and to ensure continuity of care for Veterans and Veteran families served in the program.
  • Recruitment/Relocation incentive may be paid to a Highly Qualified candidate (new to federal government or 90 days or more break in service).
