About the Role
The Purchased Non-Institutional Care/Specialty (PNIC) GEC Social Worker Program Coordinator is assigned to the Home & Community Based Services (HCBS) Department in the Rehabilitation & Long-Term Care (RLTC) Division under the Portland VA Health Care System.
What You'll Do
- →The duties of this position include, but are not limited to: Participates in the development, evaluation, and communication of standards, policies, and procedures, and plans of care for the PNIC/SPECIALTY GEC program.
- →Educates medical center colleagues and/or students regarding the PNIC/SPECIALTY GEC program and assist with dissemination of PNIC/SPECIALTY GEC information to other VA facilities.
- →Evaluates PNIC/SPECIALTY GEC practices using relevant statutes and regulations and takes action to improve identified deficiencies.
- →Tracks PNIC/SPECIALTY GEC data and identifies significant findings.
- →Tracks spending and resource utilization and makes decisions regarding resource allocation.
- →Identifies and develops program resources in the community, providing liaison with these providers.
- →Provides program oversight to ensure quality care is provided by contracted agencies.
- →Participates in research and utilization.
- →Completes an initial standardized screening of Veterans to determine whether they meet eligibility criteria for inclusion in a PNIC/SPECIALTY GEC program.
- →Develops an assessment of the Veteran in collaboration with the PNIC/SPECIALTY GEC team, the Veteran, family members, and significant others, whenever possible.
- →Reassess Veterans on a regular basis to ensure they are receiving the appropriate level of services and remain in need of program support.
- →Responsible for developing a psychosocial treatment plan and setting achievable treatment goals with the Veteran and/or family in collaboration with PNIC/SPECIALTY GEC team members.
- →Ongoing assessments and updating treatment plans are done when necessary or on a quarterly basis given the changing needs as health status changes.
- →Social workers will address the unique needs of homebound, frail, disabled, chronic, and/or dying Veteran.
Requirements

- ✓Must be proficient in written and spoken English.
