About the Role
The Care Coordination Integrated Case Management (CCICM) and COMPACT Act Coordinator is a Social Work Program Coordinator whose primary responsibility is overseeing the daily operation of these programs. The Coordinator provides Veterancentric services in each of the programs and develops policies and procedures for program operation and prepares reports and statistics for facility, VISN, and national use.

- →VA Careers - Social Work: https://youtube.com/embed/enRhz_ua_UU VA Careers - Licensed Clinical Social Worker: https://youtube.com/embed/U_xC25QsN0w Major Duties: Responsible for establishing and maintaining Care Coordination Integrated Case Management (CCICM) as a facility-wide practice change in accordance with VA guidelines and local policies.
- →This includes collaborating with Nursing and other applicable services to ensure seamless service delivery, and developing workflows, referral processes, and documentation standards.
- →Conducts assessments to determine eligibility and appropriateness for CCICM services.
- →Monitors Veteran care management and Lead Coordinator assignments to ensure compliance with program goals.
- →Responsible for tracking program outcomes and performance metrics.
- →Provides training to VA staff, community partners, and Veterans on CCICM policies and procedures.
- →Serves as the primary liaison between the VA and community-based agencies for CCICM.
- →Provides administrative oversight of the program and administers information and analytical systems to evaluate and enhance the quality of service provided to Veterans.
- →Provides assessment, treatment, and planning for a complex population of Veterans that reflect the age-specific, psychosocial, and functional needs of the Veteran with serious mental health issues during suicidal crises.
- →Demonstrates knowledge of VA and community resources to meet the complex psychosocial needs of this unique population of Veterans and to ensure they are referred to the least restrictive environment that meets the level of care needed.
- →Performs a wide range of coordination and education services, both internally and externally, and provides technical assistance to local, community partners.
- →Performs work related to the review, monitoring, analysis, planning, and assessment of facility COMPACT Act responsibilities.
- →Work Schedule: Monday through Friday 8:00 am to 4:30 pm Recruitment Incentive (Sign-on Bonus): Not authorized Permanent Change of Station (Relocation Assistance): Not authorized Pay: Competitive salary and regular salary increases.
- →When setting pay, a higher step rate of the appropriate grade may be determined after consideration of higher or unique qualifications or special needs of the VA (Above Minimum Rate of the Grade).
