About the Role
The Post-9/11 Military 2 VA (M2VA) Program Coordinator who provides program oversight and clinical social work services at an advance practice level. The duties and responsibilities relate to the care management of severely ill, polytrauma and/or injured Operation Iraqi Freedom (OIF), Operation Enduring Freedom (OEF), Operation New Dawn (OND) service members, medically retiring/transferring from Medical Treatment Facilities (MTF), and Veterans.
What You'll Do
- →Clinical Functions Posses an advanced level of skill and expertise to establish and maintain an effective therapeutic relationship with Veterans and/or their families, caregiver, or survivors.
- →Works independently with Veterans and their families who are experiencing a wide range of complicated medical, psychiatric, emotional, behavioral, and psychosocial problems using his/her education training, clinical experience, VA policies and accreditation standards in administering direct services.
- →Demonstrate use of advanced skills and sophisticated knowledge of Veteran treatment and disposition in accordance with criteria for care planning across the Veteran care continuum and in a variety of clinical settings and levels of care.
- →Demonstrate an ability to complete thorough assessments to determine the psychosocial functioning and needs of Veterans and/or their families, caregivers, or survivors.
- →Experience in making rapid assessments and developing crisis management plans for deferral of or admission or to mental health inpatient programs.
- →Provides mentorship and clinical supervision to enrich social work knowledge and skills to competently provide quality care and improved clinical outcomes.
- →Administrative Functions Attend all appropriate staff meetings.
- →Collaborate in performance improvement processes and comply with performance measures as required by the VA for their specific population and/or program.
- →Participate in the monthly Peer Review process.
- →Perform other duties as assigned.
- →Work Schedule: Monday-Friday; 8:00am to 4:30pm Telework: Available.

Requirements
- ✓You must be a U.S.
