About the Role

This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific individual eligibility requirements in accordance with VHA policy and submit your EDRP application within four months of appointment. Program Approval, award amount (up to $200,000) and eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after complete review of the EDRP application.

What You'll Do

  • The incumbent is administratively responsible for the Dom VA Medical Center Respite Services Program located at the Columbia VAMC CBOC location and serves as Lead Social Worker in Patient Aligned Care Team (PACT) for the CBOCs.
  • Total Rewards of a Allied Health Professional Major duties include, but are not limited to: The Social Work Coordinator screens Veterans for enrollment in the VA inpatient respite program and makes recommendations for non-institutional care such as CADHC or in-home respite if no inpatient respite bed can be located.
  • Incumbent is expected to demonstrate an advanced understanding of the range of treatment and skills needed for all adult patients including geriatric patients related to their physical limitations, psychological needs, and associated illnesses and conditions.
  • The incumbent must understand the problems associated with advanced aging and respect the diversification of the aging veteran population.
  • They are also expected to be sensitive to human growth and development appropriate needs and concerns of the geriatric Veteran population and able to provide current and advanced techniques for the treatment of this population.
  • Throughout treatment/enrollment, the Social Work Coordinator is the subject matter expert on VA and/or community resources and the respite program.
  • Incumbent is experienced in making rapid assessments and developing crisis management plans.
  • Incumbent provides education related to VA and community resources, entitlements, health care planning, and will refer veterans/families to appropriate interdisciplinary team members for identified health education needs if incumbent is unable to assist or is out of the social workers scope of practice.
  • Incumbent Reviews the record in CPRS and ensures all documentation is completed to facilitate the Veteran's placement to include requesting a Level of Care (LOC) and PPD.
  • Incumbent alert the assigned primary care team of any missing documentation to prevent admission delays.
  • Upon receipt of referral for in-patient respite at a Contract Nursing Home (CNH), and upon confirmation the Veteran has an appropriate LOC in CPRS (if applicable), Incumbent sends referrals to the CNHs identified by the family for possible respite bed offers; follows up with CNH Admission Coordinators as needed and resends any additional documents/information requested.
  • Once a bed offer is received, Social Work Coordinator notifies the caregiver of the bed offer(s) to obtain consent; enters the CNH consult for in-patient respite.
  • Incumbent will design a tool or create a mechanism to track data for all admissions for respite.
  • The Social Work Coordinator will provide Veterans and their caregivers with supportive counseling as appropriate.
