The VA Long Beach Health Care System is recruiting for (1) Social Worker (Program Coordinator) to coordinate the Medical Foster Home (MFH) and the Contracted Adult Day Health Care (CADHC) Programs. The SWPC is responsible for maximizing psychosocial quality of life issues to assist Veterans to remain in their home or in a less restrictive level of care other than a nursing home and avoid hospitalization. Works closely with Home-Based Primary Care (HBPC) and Spinal Cord Injury (SCI) Home Care.
What You'll Do
- →This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program.
- →You must meet specific individual eligibility requirements in accordance with VHA policy and submit your EDRP application within four months of appointment.
- →Program Approval, award amount (up to $200,000) and eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after complete review of the EDRP application.

- →They use advanced clinical training, insight, and experience to interpret data and to identify viable treatment options, including admission to an MFH Establishes a continuing relationship with the MFH Veteran and Caregiver and involved family members Acts as an advocate when it serves the best interest of the Veteran/family and/or Caregiver Work Schedule: Monday-Friday; 7:30am-4:00pm.

Requirements
