About the Role
This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific individual eligibility requirements in accordance with VHA policy and submit your EDRP application within four months of appointment. Program Approval, award amount (up to $200,000) and eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after complete review of the EDRP application.

- →Louis Health Care System.
- →Incumbent has experience that demonstrates possession of advanced practice skills and judgment in working with highly complex homeless veterans and the grant agencies.
- →The social work coordinator independently provides clinical, psychosocial and case management services at an advanced practice level to eligible veterans and their family members to meet biological, psychological, social and environmental needs.
- →Veterans treated come from a wide, multi-state geographic area.
- →Their health care and psychosocial problems and needs are complex and require a high degree of clinical oversight and creative problem solving.
- →Services are provided in an outpatient office setting and in the community.
- →The incumbent provides comprehensive social work services to the Healthcare for Homeless Veteran Grant & Per Diem Program.
- →The incumbent independently coordinate services to veterans who have extremely complex care needs due to homelessness and complicated health and behavioral health care issues.
- →The incumbent is considered a subject matter expert in the area of homelessness and the Grant & Per Diem program.
- →The incumbent provides consultation to VA staff and social workers concerning these conditions and educates incumbent has advanced practice skills working with any available family concerning the stress of caring for an individual with complex homeless issues.

Requirements

Benefits

- ★When setting pay, a higher step rate of the appropriate grade may be determined after consideration of higher or unique qualifications or special needs of the VA (Above Minimum Rate of the Grade).
