About the Role
The Social worker who serve as the Program Coordinator at the GS-12 grade are administratively responsible for a clinic program providing treatment to patients in a major specialty, such as Spinal Cord Injury (SCI) Medical Foster Home, Community Residential Care, Community Nursing Home Program, Adult Day Health Care, Respite, Suicide Prevention Coordinator, Caregiver Support Coordinator, Local Recovery Coordinator, and other specialized programs.

- →He/ She develops and implements information systems to track service activities including visits, referrals, inpatient days patient demographics, equipment utilization and patient satisfaction.
- →The Coordinator develops, implements and evaluates clinical guidelines and protocols to establish appropriate utilization of services.
- →The Social Work (Program Coordinator) performs the administrative and personal management function relative to staff supervised.
- →HS/ She establishes guidelines and performance expectations for staff, which are clearly communicated through the formal employee performance management system.
- →The Social Worker establishes and maintains effective therapeutic relationships with Veterans and their families.
- →The Social Worker acts as a subject matter expert and liaison with community agencies.
- →He/ She coordinates community-based services, including information and referral to or from other VA programs, government programs, and community agency programs.
- →The Social Worker has knowledge and experience in the use of medical and mental health diagnoses, disabilities, and treatment procedures.
- →The Social Worker (PC) identifies high-risk patients and provides appropriate intervention case management services.
- →Work Schedule: Monday - Friday, 8:00am to 4:30pm.
- →Subject to change due to the needs of the facility.
- →When setting pay, a higher step rate of the appropriate grade may be determined after consideration of higher or unique qualifications or special needs of the VA (Above Minimum Rate of the Grade).
- →Paid Time Off: 37-50 days of annual paid time offer per year (13-26 days of annual leave, 13 days of sick leave, 11 paid Federal holidays per year) Selected applicants may qualify for credit toward annual leave accrual, based on prior [work experience] or military service experience.
Requirements

- ✓Must be able to independently operate Government Vehicle.
