About the Role

The Social Worker Program Coordinator serves as the professional leader of the Primary Care Behavioral Health (PCBH) Program and reports to the Section Chief of Behavioral Medicine. PCBH is co-located with the Primary Care medical teams, with an emphasis on integration of mental health services within primary care. The incumbent is directly responsible for the planning, development, implementation, monitoring, and evaluation of treatment program operations.

  • Responsibilities include the development and implementation of programs, policies, and procedures; oversight of administrative and programmatic resources; and monitoring of outcomes.
  • The incumbent performs the administrative and personnel management function relative to staff supervised including personnel actions, clinical supervision, consultation, negotiation with other departments, and quality improvement.
  • Works collaboratively with internal staff, VISN and VACO leadership, and external stakeholders to expedite service delivery.

  • As a full clinical member of the PCBH Team, provides a full range of clinical services appropriate to the primary care setting, especially in the areas of diagnostic and psychosocial assessment and evaluation of emotional and behavioral problems.
  • Provides short-term individual, family, and group therapy services in accordance with the needs identified in the assessment and treatment plan.
  • Conducts mental health assessments, utilizes standardized instruments in the assessment of mental health conditions, and makes DSM-5 diagnoses, screens for substance abuse, tobacco use, and mental health disorders.
