About the Role

The incumbent serves as the Program Coordinator for the Medical Foster Home (MFH) Program, providing both clinical and administrative oversight of Veteran care and program operations. Responsibilities include managing the daily operations of the MFH program, delivering evidence-based clinical services and interventions.

What You'll Do

  • Total Rewards of a Allied Health Professional Duties include but not limited to: The incumbent is administratively responsible for the overall functioning of the Medical Foster Home (MFH) Program and oversees all aspects of the program.
  • Responsibilities include recruitment and development of the medical foster home caregivers and recruitment and placement of veteran residents into the homes; development of policies, procedures, and care processes; and coordination with Home Based Primary Care (HBPC) interdisciplinary treatment teams.
  • The incumbent completes initial, annual, and focused inspections of the medical foster homes and potential caregiver homes while maintaining professional collaboration when working with both internal and external stakeholders.
  • Utilizes Microsoft Word and Excel for office automation including storing and retrieving electronic documents and uses VistA and the Electronic Health Record (EHR) to access patient information and data.
  • The incumbent provides assessment, treatment, and planning for a complex population of patients which reflects the age-specific, psychosocial, and functional needs of the patient (e.g., spinal cord, geriatric, and mental health population).
  • The incumbent provides case management to Veterans requiring substantial communication skills, organizational skills, problem-solving and time management.
  • The incumbent utilizes various counseling approaches to include, but not limited to, conflict resolution, anger management, and effective/healthy coping skills.
  • The incumbent bases these decisions on advanced practice skills, professional judgment and the expertise regarding the Veteran's self-care capability, the family support system, the Veteran's health care needs and possible consequences.
  • The incumbent assists and encourages Veterans and significant others in facing problems, thinking them through, evaluating the situation realistically, considering alternative courses of action, and arriving at plans for using resources to resolve problems.
  • Documents all social work activity in the patient's medical record as appropriate.
  • Maintains accurate statistics, including Outpatient Encounter Forms, and prepares other reports, as required, within specified deadlines.
  • Submits written and/or enters electronic data promptly and accurately per policy and/or procedures set by Social Work Service, the Medical Center, VISN, VA Central Office, The Joint Commission, MFH National Office and other entities as appropriate.
  • Incumbent completes suicide risk screens C-SSRS, CSRE, and Safety Plans, completing and documenting assessments within the timelines and guidelines outlined in local policy and guidelines (i.e., VANTHCS Suicide Prevention Standard of Work) as required and appropriate.
  • The position requires frequent travel away from the duty station to see Veterans in medical foster homes, to screen potential veterans for placement in medical foster homes, and to assess potential medical foster homes and caregivers in the community.
