About the Role
This position serves the Inpatient Social Worker at the Central Arkansas Veterans Healthcare System (CAVHS) and provides a high level of skill in assessing and treating the complicated psychosocial problems of Veterans. Weekend and Weekday positions.
What You'll Do
- →Identifies Veterans at elevated risk (e.g., frequent admissions, lack of social support, homelessness, substance use, guardianship needs) and implements targeted interventions in conjunction with the healthcare team to reduce avoidable readmissions and improve health outcomes.
- →Completes an appropriate (brief or comprehensive) psychosocial assessment to determine the psychosocial functioning and needs of Veterans and/or their support systems.
- →Develops an individualized discharge/care transition plan coordinating with the healthcare team, community resources, VA and Non-VA services, and families.
- →Performs appropriate, timely, and effective suicide risk screenings and comprehensive suicide risk assessments.
- →Develops and integrates safety planning and safety goals into the Veteran's collaborative, individualized treatment plan.
- →Participates actively in daily interdisciplinary care team meetings, offering expert input on psychosocial factors and case management needs impacting clinical decision-making and care planning.
- →In conjunction with the healthcare team, identifies and assesses risk, safety, and supports, and implements appropriate interventions/treatment for those who are experiencing abuse, neglect, exploitation, or domestic or intimate partner violence.
- →Adheres to VA and mandated reporter requirements and advises and collaborates with interdisciplinary teams throughout the medical center on these issues.
- →Is proficient in using medical and mental health diagnoses, disabilities, and treatment procedures, including acute, chronic, and traumatic illnesses, and medical terminology.
- →Provides Advanced Directive education to Veterans/support systems and assists with completing the appropriate forms.
- →Also assists the interdisciplinary team with identifying appropriate decision makers if the Veteran is unable to make their own decision (Next of Kin, HCPOA, Legal Guardian).
- →Facilitates advanced care planning group visits with Veterans and support systems as appropriate.
- →Collaborates with Veterans/support systems and the interdisciplinary treatment team to ensure that appropriate VA and community placement referrals are completed in a timely manner.
- →Placements or referrals can involve, but are not limited to, Contract or Community Nursing Homes, Arkansas State Veterans Home, Community Living Centers, Residential Care Homes, Medical Foster Homes, Respite Care, Assisted Living Facilities, and Inpatient and Outpatient Hospice Services.
