About the Role

This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific individual eligibility requirements in accordance with VHA policy and submit your EDRP application within four months of appointment. Program Approval, award amount (up to $200,000) and eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after complete review of the EDRP application.

What You'll Do

  • Position vacancies are at Gainesville VAMC and Jacksonville locations.
  • This position is located at the North Florida/ South Georgia Veterans Health System in the Community Reintegration and Recovery Service.
  • The incumbent provides community-based case management services to Veterans participating in the HUD VASH program.
  • Works closely with the VA medical center's homeless programs, residential and community programs, and domiciliary programs to ensure that coordination between these entities is facilitated and Veterans direct care, referral, and follow-up is based on a continuum of services.
  • Independently provides clinical psychosocial and case management services for eligible Veterans and their family members in the program.
  • Works in close consultation with the HUDVASH Social Worker assigned.
  • Veterans eligible for HUDVASH come from a wide geographic area.
  • Their health and mental health needs may be complex and require a high degree of clinical oversight and creative problem solving.
  • Services are provided in healthcare settings and in the community.
  • The HUDVASH Program requires continuous coordination with the community's homeless providers and coalitions as well as with other local, state, and federal agencies.
  • Duties of the Social Worker - HUD-VASH program include but are not limited to the following: Develops working relationships and agreements with other organizations and directly coordinates with the designated Public Housing Authority(PHA) regarding housing voucher application and award process for each veteran.
  • Regularly conducts and participates in outreach activities including field interviews, assessments and referrals for homeless veterans contacted in the community, shelters, and "on the streets," as well as those who are referred by VA residential programs, medical centers or outpatient clinics.
  • Evaluates the veteran's situation, abilities, and capabilities, and arrives at a reasoned conclusion including an assessment of vulnerability and priority for admission.
  • Assesses at-risk factors and develops a preliminary plan, involving the veteran and family or significant others, and performs an insightful assessment of serious and complicated cases involving psychiatric illness which may also include, catastrophic medical condition, dementia, traumatic brain injuries, and other high risks diagnoses.
