Social Worker - HUD VASH

About the Role
This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific individual eligibility requirements in accordance with VHA policy and submit your EDRP application within four months of appointment. Program Approval, award amount (up to $200,000) and eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after complete review of the EDRP application.
What You'll Do
- →Total Rewards of a Allied Health Professional Formulates and implements treatment plans in collaboration with the Veteran, interdisciplinary team, and when appropriate family members or caregivers.
- →Utilizes supportive counseling, problem-solving strategies, and crisis intervention techniques to assist Veterans experiencing psychiatric, medical, financial, legal, and social problems.
- →Monitors treatment progress and reassess Veteran needs at least annually or more frequently as clinically indicated.
- →Ensures treatment plans include strengths, measurable goals, referral plans, and relevant history including mental health, substance use, medical, legal, housing, and social functioning.
- →Provides housing stabilization interventions designed to support long-term tenancy and prevent eviction, including problem-solving with Veterans regarding lease compliance, conflict resolution, and engagement in supportive services.
- →Delivers crisis intervention and supportive counseling to Veterans experiencing housing instability, behavioral health crises, or other psychosocial stressors impacting housing retention.
- →Assists Veterans in developing skills necessary for independent community living, including budgeting, maintaining housing, navigating community systems, and engaging in healthcare and recovery services.
- →Provides case management and care coordination to facilitate appropriate delivery of healthcare services.
- →Conducts face-to-face meetings with Veterans in their homes or community settings as clinically appropriate.
- →Collaborates with interdisciplinary teams, VA services, and community partners to ensure coordinated care.
- →Maintains a network of community resources and referral options to support Veteran stability and recovery.
- →Maintains collaborative working relationships with Public Housing Authorities (PHAs), landlords, property managers, and housing program staff to support successful implementation of the HUD-VASH voucher program.
- →Coordinates with community housing providers and supportive service agencies to ensure continuity of care and access to resources necessary for housing stability.
- →Facilitates communication between Veterans, VA providers, housing authorities, and community partners to resolve housing barriers and support Veteran engagement in services.
Requirements
