About the Role

This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific eligibility requirements per VHA policy and submit your EDRP application within four months of appointment. Program Approval, award amount (up to $200,000) & eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after review of the EDRP application. Former EDRP participants ineligible to apply.

What You'll Do

  • VA Careers - Social Work: https://youtube.com/embed/enRhz_ua_UU Major duties include but not limited to: Implement treatment modalities in working with individuals, families, and groups to achieve treatment goals.
  • Formulate a treatment plan and implement the plan in conjunction with the interdisciplinary team, Veteran and if appropriate family members/caregivers.
  • Establish and maintains effective working relationships and communicate with Veterans, staff, and representatives of community agencies.
  • Provide case management and care coordination to facilitate appropriate delivery of health care services, act as an advocate with appropriate VA and community service providers/agencies when it serves the best interest of the Veteran and family members/caregivers.
  • Coordinate face to face meetings take place in Veteran's residence, unless otherwise indicated.
  • Refer Veterans with services, resources, and opportunities both in the VA and community, to maximize the Veteran's independence, health, and well-being of the Veteran.
  • Communicate with Veterans, staff and representatives of community agencies to build collaboration and meet the needs of Veterans.
  • Maintain a current network of internal and external resources to educate Veterans, family and team members and assist with appropriate referrals.
  • Make appropriate referrals to community and other agencies, coordinating services as needed.
  • Leverage comprehensive knowledge of community resources to effectively support veterans in need.
  • Ensure Veterans receive specialized care beyond what the VA can provide.
  • Collaborate closely with community partners and government agencies, such as local health departments, housing authorities, and nonprofit organizations.
  • Develop and implement personalized service plans that address housing stability, mental health support, substance abuse treatment, vocational rehabilitation, and other essential services.
  • Foster interdisciplinary collaboration among healthcare providers, case managers, and community stakeholders, and ensure holistic care that aligns with the VA's mission to promote Veterans' recovery, independence, and quality of life.
